• Medical condition of the patient, including:
  - Date of accident or onset of illness
  - Current medical condition
  - Age and weight of patient
  

• Destination
  - Name of preferred Doctor
  - Name of preferred Hospital
  - City / town / village
  - Country

This information will assist Mediflight’s experienced staff to determine the best method of transfer for the patient.
